The first episode was a double episode leading to different sources to count differently. As RMT pulls from various sources this results in duplications and occasionally an apparent omission.

The Previous episode S03E11 was Chapter 37, now this one S03E12 and S03E13 shows Chapter 39.

Actually, according to IMDB, there are only 12 episodes in season 3.
